A veterinarian dealing with family emergencies forgets to renew a premises license

Arthur's premises license lapsed and the veterinary board reached out to him without success. He called years later to renew the premises license for the new renewal cycle but discovered his premises no longer existed. The Board voted to open an investigation and several days later it appears Arthur was able to renew his premises license.

Arthur's response tells us that he initially thought his personal veterinary license was tied to the premises license but later learned that wasn't the case. He prepared the correct filing but believes he neglected to send it because of deaths and medical emergencies in the family. He's very contrite and takes full responsibility several times in his response.

Arthur was required to pay a $1000 fine.
