Two dogs get caught up in a billing problem between a complainant, a vet, and a rescue

The complainant says he contacted Arizona Spay Neuter on behalf of a low-income disabled veteran and his pets. The dogs both needed dentals, one needed a neuter, and one needed a mass removal. The cost was to be borne by charity organizations and someone at the clinic also recommended another organization that could help. The owner pitched in $100 toward the cost and the dogs were brought to the clinic. Sadly it appeared that the clinic had no idea what was to be done and seemed confused. He also attributes problems at discharge and with the billing to the clinic (though it also reads that perhaps the problem was with the various organizations, it's hard to tell if the clinic's getting blamed for problems beyond their control as well as their own). We're also shown an email detailing alleged problems with the dogs' medical records, and there was also a dispute about a final invoice and attempting to prescribe the dogs pills instead of injections.

Twiddy's response is short and says that she had no direct contact with the complainant in this case. She says that the complainant selected a hernia repair rather than a mass removal (in his complaint he says a staff member told him to do that).

This was an Investigative Division report so there's no discussion from a committee. The Findings of Fact don't go into any particular evaluation of the claims about bad medical records. It also appears that the complainant, not the rescue, was banned from the premises in the future. We also learn that the mass was removed without a charge but no testing was done on it, ostensibly because of cost issues; reading the summary, it appears that the main confusion was whether or not the mass removal and biopsy was going to be covered by somebody else.
