The complainant got a new cat and took him to the vet for his shot. She then reviewed the paperwork when filling out a pet insurance application and discovered that the vaccines were expired when they were given. She says that she's contacted Alta Vista for help and wants a refund or scheduled for vaccines closer to home now that they've moved. Alta Vista allegedly spoke with her and said the best they can do is to send an updated invoice, but the complainant apparently thinks that they're just covering for the fact they didn't do it right the first time.
Jackson was the supervising veterinarian when the cat got his vaccine. She says that in the medical record the sticker for the vaccine clearly shows that it had not expired. She's not sure how this could have happened but says that it's only a matter of the paperwork being wrong, likely as a result of a clerical error; perhaps the wrong date was entered into a computer.
The Investigative Committee said that it was just a paperwork error and that the cat received acceptable vaccines. They also said the premises had implemented new policies to make sure clerical errors like this don't happen again. The Board sent a letter of concern related to medical records.
